I found Apache Junction health centre to be very well or, clean, and especially focused on good relationships between staff and patients. I was admitted for a fall from the medical surgical hospital and the transition went very smoothly. Everyone seems very caring.
My sister and I visited the facility for our mother. Kevin was great! He answered all of our questions and put us at ease.
The therapy team at this facility goes above and beyond. They are extremely knowledgeable and demonstrate incredible empathy towards all their patients.

I am writing this review out of deep concern for the care my father is currently receiving at this facility. Unfortunately, repeated attempts to address serious issues directly with management have gone unanswered, leaving our family with no choice but to bring public attention to these matters. Over the past few months, multiple failures in care and communication have occurred, including: • A written report regarding a staff member attempting to gain access to my mother’s vehicle. This incident was formally reported via email to Administrative Manager Kaylee and Director Ian, yet over a month later, there has been no acknowledgment or response. • My father went without a prescribed air mattress for over 30 days, placing him at risk for serious medical complications. After returning to this facility after his hospitalization. • Therapy services that were promised prior to admission have not been consistently provided. As a result, my father has experienced a noticeable decline in strength while awaiting prosthetic services. He’s only had 3-4 physical therapy treatments in the span of almost 4 months in this facility. • Meals are not being served consistently three times per day as expected in a rehabilitation setting. I call my father where it’s been past 6pm and has received no meal. • Most concerning, my father has been left sitting in his own feces for extended periods of time, which is unacceptable and a direct violation of basic standards of care, dignity, and patient rights. One day I was on lunch where his light was left on for assistance and by the time I was off of work he was still in his own feces. My father is an amputee and is not mobile. • Physicians have repeatedly had difficulty reaching staff at the facility to schedule my father’s necessary medical appointments. Calls from physician offices have gone unanswered, resulting in missed and delayed care. • My father has missed critical appointments due to the facility’s failure to coordinate care, including an X-ray related to his amputation, prosthetic services, and dermatology treatment for his bullous skin condition, which requires ongoing medical management and injections bimonthly. With this condition going untreated it cause huge open sores. These issues have been documented through emails, phone calls, and voicemails. All supplied by their frontline staff member Melinda. I will attach a Screenshot and it will be attached to support that I have gone acknowledged by manager and director of this facility. At this point, the lack of response from facility leadership—specifically Administrative Manager Kaylee and Director Ian—demonstrates a troubling disregard for patient care, coordination, and family communication. If immediate corrective action is not taken, I will be formally reporting these concerns to the appropriate regulatory and oversight agencies, including the Arizona Long-Term Care Ombudsman Program and the Arizona Department of Health Services. I will also be requesting a full review with the insurance provider to verify that all billed services were actually performed and properly documented. My goal is not conflict. My goal is safe, respectful, timely, and appropriate care for my father and every other resident in this facility. Families trust rehabilitation centers during vulnerable moments. That trust must be honored through accountability, responsiveness, and proper care. BEWARE of their reviews, Director in this email just gave theirselves 5 stars for the team they have onboard. I’d give them zero if possible. We placed my father, who has Alzheimer’s disease, at Apache Junction Health Center for a 30-day respite stay paid for by the VA while my mother and I recovered from surgeries in January 2026. Unfortunately, our experience was deeply concerning. A formal complaint has been made with ADHS and is under investigation. Despite providing all of his medications upon admission (including detailed instructions), we later discovered that one of his prescribed antidepressants (Sertraline) was never administered during his stay. Multiple staff members contacted us about his severe depression and uncontrollable crying, even suggesting adding new medications—without us realizing he wasn’t receiving the one he was already prescribed. Communication was poor. We were told we’d receive follow-up calls from nursing staff and the facility physician, but those calls never came. This left us making critical decisions without accurate information. By the time he was discharged, his mental state had significantly declined and he is still not recovered from this. He was far more withdrawn and cognitively worse than when he arrived. To make matters worse, several of his personal belongings—including hearing aids, eyeglasses, and clothing—were lost. His discharge papers indicated he had no hearing loss/aids, yet the staff admitted they could not locate them. Incredibly frustrating for my dad. The facility was unresponsive in addressing our concerns about my father’s medication after discharge. While I was able to get my dad to an audiologist in early February, the VA was not able to provide new hearing aids until mid-March, so my father was affected for about 10 weeks. If you know anything about dementia and hearing, you understand how traumatic this was for all of us. We would strongly urge other families to carefully consider their options before placing a loved one here. I would NEVER recommend this facility. The guilt of placing a loved one with someone beyond family is tough enough, so to have endured the lack of care with this VA-recommended facility is heartbreaking.
Apache Junction Healthcare Center has a team of caring team that makes sure your loved one gets back to their prior level of care through their physical therapy team and great nursing staff. 5 star quality!!!
My father needed care after a hip surgery and came out walking which surprised all of the family. Great job.
Good physical therapist. Poor nursing care. Don’t get ill when you are here. You might not survive
Went to see our friend! We went to commons area! Place was clean and we enjoyed ourselves!
